Services we provide
Our older adults and home health AT&R service provides a specialist inpatient service dedicated to improving and maintaining the health and independence of older people.
The service is a mix of geriatric medicine and rehabilitation for older people who often have multiple or complex needs.
We provide care and rehabilitation for older people including those recovering from an injury, illness or age-related conditions such as stroke or Parkinson's disease.
We can help you tackle problems associated with disability or ageing that affect your daily living.
We work with you and your family to plan your care and help you achieve your healthcare goals. Depending on your issues and goals, your team may include:
- doctors
- nurses
- physiotherapists
- occupational therapists
- speech-language therapists
- social workers
- needs assessors.
Referral information
You can be referred to AT&R by:
- your healthcare provider or other health professional involved in your care
- acute hospital services.
