Get a copy of your immunisation record

You may find it useful to have a copy of the immunisation record for you or your tamariki (children). For example, for school enrolment, B4 School Checks, boarding school or halls of residence, employment checks or when travelling overseas. If you have completed your My Health Account identity checks to allow you to fully access My Health Record, this may be the easiest way to get a copy.


Ways to get your immunisation record

Use My Health Record

  • Sign up for My Health Account to access your immunisation records in My Health Record.
  • You need to be age 16 or older and have a valid New Zealand or Australian identity, for example a passport.
  • Once you are logged in, you can view your immunisation record and download it.

Make an online request

  • Request your immunisation record using our online form.
  • You do not need a My Health Account to do this.
  • You do need to supply personal information to verify who you are.
  • You do need to provide an email address so we can email the record to you.
  • We aim to respond within 1 to 5 working days.

Phone the contact centre

  • My Health Record has a contact centre for more immediate support.
  • Call Monday to Friday, 8am to 5pm on freephone 0800 222 478 — you may need to wait at busy times.
  • You need to provide enough information to allow us to verify your identity over the phone.
  • We can email your record to you while you are on the phone. 

Ask your healthcare provider

  • Some medical centres, pharmacies and other healthcare providers can print off your immunisation record.
  • There might be a cost for doing this, so it is a good idea to ask first.

  • These requests can take up to 20 days to process.

Ways to get an immunisation record for your tamariki

Use My Health Record

  • Sign up for My Health Account to access immunisation records in My Health Record.

When you have upgraded your My Health Account to 'Identification level: Complete', you may be able to use My Health Record to link in the immunisation records for your tamariki if:

  • they are under 12
  • you are older than 16
  • your name is on their birth certificate.
