My Health Record
My Health Record gives you secure access to some of your health information, even if you are not enrolled with a healthcare provider.
Parents of children under 12 (with a New Zealand birth certificate) will be able to link their child and view some of their health information, including their immunisation records.
We are continuing to grow and offer better access to your records. Over time, more information and functionality will be made available.

What you can do in My Health Record
In My Health Record you can:
- view your immunisation records dating back to 2005
- view your COVID-19 test results
- report a COVID-19 test for yourself or someone else
- view your information on the NHI system, and update your name, address, ethnic group and gender
- view your active High Use Health Card and Community Services Card entitlements
- view your linked children's immunisation records and some NHI details.
Why we need My Health Record
Supporting people to use digital services to access their health information is an important part of health sector reform in Aotearoa New Zealand. Currently, your health information is not captured in one place. My Health Record provides you with a way to see all your health information in one central place.
- It lets you have better access and control of your healthcare information — whether or not you are enrolled with a healthcare provider.
- It lets you take greater responsibility for your health by getting you more involved in managing your healthcare.
- It will give more people the opportunity to manage their health in their homes and communities.
How My Health Record is different to other 'patient portals' or apps
Many healthcare providers offer their patients the use of a 'patient portal' or app for their GP Practice Management System, such as MyIndici and Manage My Health. My Health Record will complement these systems and is not intended to replace them.
While some people can access parts of their health information through a patient portal provided by their healthcare provider, not everyone has access to a patient portal. My Health Record helps to provide you with a way to see your health records that are held on central systems, whether or not you have access to a patient portal.
How to access My Health Record
You will need:
- a device with an internet connection
- an email address that is unique to you and not shared with anyone else
- a valid New Zealand or Australian identity document, such as a drivers licence, the name of the GP practice or medical centre you are enrolled with, or both
- to be aged 16 years or over.
Getting set up
- Go to My Health Record
- Create a My Health Account to sign up.
- Enter your email address and create a password.
- Verify it is you by either entering the details of a valid New Zealand or Australian identity document, the name of the GP practice or medical centre you are enrolled with, or both.
Upgrading your account
When you first log in to My Health Record, your account will be set to 'Identification Level: Basic'. You will only be able to see COVID-19 information and your NHI information.
You will need to upgrade your My Health account to see other heath information, such as your wider immunisation history, and to update some NHI details.
You can upgrade your account by selecting the 'Confirm identity in My Health Account' which will take you through the upgrade process.
My Health Record privacy and data
My Health Record does not hold any data. But it gives you secure access to view personal health data that is stored in Health New Zealand's systems, such as the:
- Aotearoa Immunisation Register
- laboratory test results systems
- National Health Index system.

My Health Account and My Health Record help
Some people may be unable to upgrade their My Health Account.
For some people, a match to a healthcare provider may not be found. This could be because you have changed your phone number since you enrolled with a GP, or you may not be currently enrolled with a GP.
If you are having trouble upgrading your My Health Account, you can:
- call the Health New Zealand contact centre on 0800 222 478, Monday to Friday, 8am to 5pm
- email support@identity.health.nz.

About My Health Account (external link)
If you are not enrolled with a GP, you can use RealMe to create and update your My Health Account to access My Health Record.
If you do not already have a RealMe account, visit the RealMe website to apply. If you want to use RealMe to sign up for My Health Account, it is best to sign up for RealMe first.

My Health Account (external link)
If your My Health Record is set at 'Identification level: Basic' you will only be able to see your COVID-19 immunisations.
To see your immunisations dating back to 2005, you will need to upgrade your My Health Record to 'Identification level: Complete'. Instructions on how to upgrade your My Health Record can be found in the user guides on this page.
If you need support in viewing your immunisation records, you can contact us for help:
You can use My Health Record to have your immunisation records and those of any tamariki (children) linked to your account emailed to you.
An electronic document (pdf) of the immunisation records will be emailed within 24 hours.
Check your 'junk email' folder if you have not received the email after 2 business days.
Some parents can link their children’s account to their own account, to see their children's immunisation history in My Health Record.
Parents who are over 16, and whose My Health Record says 'Identification level: Complete', can link their tamariki who are under 12. The parent’s name will need to match the name on the child’s New Zealand birth certificate.
Once you have logged in to My Health Record and have upgraded your account, select the 'Add a child' link and follow the process to link the immunisation records of your tamariki.
There will be parents who are unable to add their children’s account to their own as they may not meet the matching criteria set out in My Health Account. You will not be able to link to your child’s account if:
- your name does not match what is recorded on your child’s birth certificate
- the child has an overseas birth certificate
- the child is being raised by someone other than their birth parents, for example through whāngai
- the child is 12 years old or over.
We know that the 'Add a child' feature will not be available for some parents and caregivers. Health New Zealand will continue to make improvements over time to develop this feature.
My Health Record does not hold any data. It gives you secure access to your personal health data held on the secure systems of Health New Zealand.
