The home team aims to help support you at home after leaving the hospital, or act as a support service at home to avoid being admitted to hospital.

Where to find us
The home team are available in the greater Invercargill and Dunedin areas (within 40 minutes' drive).
Services we provide
A healthcare professional from The home team will visit you in your home to find out what level of support you need to maintain your independence and remain safely at home.
The home team will support you for 2 weeks and discuss any concerns you may have. If you still need support after 2 weeks you will be referred to other community teams, services, or providers.
The service accepts support for people over 18 years old, but the majority are older people.
Who to call if you become unwell while being support by The home team
If it is between:
- 8am and 8pm (day to evening) — call us so we can make sure you are getting the right care at home
- 8pm to 8am (night) — call your main healthcare provider, Healthline on 0800 611 116, or 111 in an emergency.
Is going home the right choice
For some older people, continuing to live at home is their preferred option if there are the right supports in place.
Our 'Home as my first choice' programme starts from the position of 'What would we need to do to make staying at home possible?' For others, returning home may no longer be the right option.

Referral information
If you have been an inpatient or have visited the Emergency department one of the Home Team will visit you in your home within 24 hours of leaving the hospital.
