Hoki ki te Kāinga — early supported discharge
Hoki ki te Kāinga is a supported discharge service for people who could benefit from a short period of intensive rehabilitation in their own home to help get home from hospital and recover more quickly.

Services we provide
The Hoki ki te Kāinga service is aimed at older people — Māori and Pasifika over 55 and Pākehā over 65. The service can be offered to younger people if appropriate. The programme averages 4 weeks but can be up to 6 weeks depending on need.
The programme supports individuals to:
- improve their health and wellbeing
- regain their confidence
- increase their independence and function at home.
A staff member will visit you within 24 hours of when you return home to complete an initial assessment and to set up a rehabilitation care plan. The rehabilitation is primarily delivered by the rehabilitation assistants, who can visit up to 3 times a day. The rehabilitation programme is reviewed weekly at the minimum.
The Hoki ki te Kāinga team is made up of:
- registered nurses
- occupational therapists
- physiotherapists
- social workers
- Kaiāwhina
- rehabilitation assistants.
Referral information
People in hospital who think they may be eligible for rehabilitation at home should discuss the option with the doctors or nurses on their ward.
