Needs Assessment Services Co-ordination — South Canterbury

Needs Assessment Services Co-ordination (NASC) organisations are funded by Health New Zealand | Te Whatu Ora. Our role is to assess your eligibility for support services based on your needs. From the assessment, we may be able to allocate and co-ordinate support services for you. As a NASC agency, we coordinate the services but do not provide them.

Services we provide

Needs assessment service co-ordination is about planning your care based on your goals and support needs.

When assessing your support needs, we use a range of tools from within the InterRAI series:

  • contact assessment
  • HomeCARE assessment
  • palliative assessment
  • mental health assessment.

Following this, we may be able to allocate and co-ordinate support services to meet the needs we identify in the assessment. 

We share information with everyone who helps care for you. We work collaboratively with you and your whānau. We can:

  • help you access community care and support services
  • talk with health and community care providers
  • plan what services you might need in the future
  • provide education and support for you and your carer.

NASC works with people who have:

  • older people's needs
  • disability needs
  • mental health needs.

Referral information

For older persons or disability needs, you can refer yourself to NASC for South Canterbury.

Referrals are handled through the Integrated Community Assessment Treatment Team (iCATT) using the contact details above.

If you have a mental health diagnosis, you will be under the care and management of a psychiatrist. Self-referrals to NASC cannot be accepted for mental health clients.
