Integrated Community Assessment Treatment Team (iCATT) — South Canterbury
The iCATT team provides a single point of entry for all community referrals — one consistent process for all primary healthcare and community health services. Where possible, we try to arrange flexible approaches for your assessment and treatment. Our aim is for you to maintain your independence, be able to care for yourself and remain part of your community environment.

Services we provide
The purpose of iCATT is to enhance your ability to self manage. We provide a service that is:
- integrated
- user centred
- restorative
- culturally appropriate
- responsive.
The aim is to maintain or enhance your functional ability, health and social connections within the community. You will be supported to maximise your independence, care for yourself and take participate in your community for as long as this is safe for you and those caring for you. Where appropriate, you will be supported to take part in activities to maintain or improve mobility, strength and balance.
Where possible, we offer flexible approaches to your assessment and treatment, characterised by brief and early interventions.
Features of the iCATT service are:
- a single point of entry — one consistent process to help you access all primary and community health services
- integrated interdisciplinary care across primary and secondary care — including, district nursing, palliative care, allied health, community and Needs Assessment Service Coordination (NASC), community nursing services within the multi-condition hub
- a defined workforce development tool that matches service need to staff competencies through implementation of the Calderdale framework
- a clear philosophy behind our care models, such as restorative care, rehabilitation and self-management
- case management for people with complex needs
- targeted approaches to the individual's needs to promote the use of consistent evidence-based interventions
- ensuring care is targeted to people who need it the most.
Referral information
You can refer yourself to iCATT by phone on the number above, or ask your healthcare provider to refer you.
General practitioners and nurse practitioners should refer as normal through the electronic request management system.
Referral forms for inpatient and community service requests are available — completed iCATT forms can be emailed to icatt@scdhb.health.nz
