Consumer Experience Survey – Chronic Kidney Disease Care Services
This survey is part of a wider project aimed at improving care for people and whānau living with Chronic Kidney Disease (CKD) in Aotearoa New Zealand. CKD is a long-term condition that affects more than 1 in 10 New Zealanders and requires ongoing care across both primary and hospital-based services. Your experiences and insights will help us understand what matters most to you, how well current services are meeting those needs, and where improvements are needed.
Use of Information
All information gathered through this survey will:
- Be used to inform programme planning and service improvement for Chronic Kidney Disease (CKD) care across Aotearoa New Zealand.
- Only be reported in an aggregated and anonymous format.
- Not be used for any other purpose.
Health New Zealand | Te Whatu Ora will not contact you unless you have explicitly indicated you would like to be contacted for follow-up purposes related to this project.

This information will be held securely by Health New Zealand | Te Whatu Ora. We will retain your information only for as long as needed to fulfil the purposes outlined in this statement and in accordance with legal obligations.
Your responses will be used to help improve the design and delivery of CKD care services across both primary and hospital-based systems. Any information you provide will be anonymised before being shared in reporting or used in programme planning.
We may share non-identifiable data with external partners (e.g., the Ministry of Health, Health Quality & Safety Commission, or relevant research bodies) for service evaluation and improvement purposes.
