Community rehabilitation service

The community rehabilitation service is for people in the Mid-North community aged 16 years and over who have had a recent medical event such as a stroke, an amputation, or any other medical condition that has caused a change in how they manage day-to-day life.

Services provided

The immediate team includes a physiotherapist and occupational therapist who can visit you in your home to develop a rehabilitation programme. 


Referral information

The service is accessed via referral from your healthcare provider, or a hospital if you have had a recent medical event.
